ST-4 - This form is used when the purchaser intends to resell the Electrical Products or Service in the regular course of business.
ST-5C - This form is used when you the purchaser intends to install the Electrical Products or Service for a job of a non-profit
organization. In conjunction with this form filled out, we require an ST-2 form of the non-profit organization.
ST-12 - This form is used for contractors and subcontractors purchasing building materials and supplies that are to be used in various tax-exempt projects.
